Registration Instructions
1. Print the forms on this page and bring to registration.
2. First contact each tutor for registration approval. Bring checks to make one to three payments for each tutor. Complete one student registration sheet per student.
3. Bring completed forms and registration fees.
Registration Fee: $50* per student with family cap of $150. This fee may be paid by check.
Building Fee: $60 per student. Make check out to Bellevue Baptist Church.

MHT Enrollment Form (one per family)
Bellevue Medical Release Form (one per family)
Goals and Objectives (one per family, initialed by each student, send only the last page)
*Registration fee based on date of registration. (See Home page)
